Preliminary Application
To the left of this panel you will see the application form. It is called "Preliminary Application" and it is a writeable PDF document. Please read the instructions on this application carefully. Type your information into the boxes and then print the document. It may be possible to save a copy on your hard drive depending on your PDF software. Sign the form, have your parents sign it also and add the required information (photo and your most recent school report card). Applications must be submitted by October 15th.

Students who are selected by the Local Rotary club will then be asked to complete the Rotary Youth Exchange Long Form. This form will be sent to you if you are a successful candidate. This application is to be completed with your sponsor Rotary Clbu and is due by December 15th.
